Notes on Plan Descriptions

►  When you click on the Learn More link, you'll be taken to a page where you can Search for Providers.

►  If your physician is not shown in the network, they can be added by asking them to call Member Services at 1-800-290-0523, or ask them to send an email with ID info to provider@careington.com.

►  Sample Fee Schedules can be obtained by locating a participating provider and clicking on the Accepting Plan: POS link in the Names column.

►  Complete fee schedules are best verified by calling your provider or Member Services at 1-800-290-0523.
